Oregon Hospital Report Outlines Security Flaws After Two Patients Escape

Published: September 16, 2008

SALEM, Ore. – After two psychiatric patients escaped from Oregon State Hospital in July, hospital officials have found multiple flaws with the facility’s security policies and are offering 22 recommendations for improvement, according to Statesman Journal.

Recommendations include adding security staff, adding more patrols, upgrading technology and training for all hospital staff.

Additionally, hospital officials determined that there was/were:

  • inconsistencies on when to notify city and state police in the event of an emergency
  • minimal security training for hospital staff
  • security cameras lacking recording technology
  • a deficiency in the patient risk review process that would evaluate patients who might escape the facility.

On July 11, patient Michael Sands overpowered hospital security to flee the premises. He was recaptured the following morning. Five days later, Mitchell Grigsby ran off the grounds. Grigsby was caught minutes after his escape.
